FIRST VISIT
Prior to your visit, please print and complete the New Patient Forms. Be sure to bring the completed forms with you to your first visit. If you are unable to print and complete the forms prior to the visit, no need to worry! Just plan to arrive a half-hour early for your appointment so you may complete them at that time.
Please also bring the following with you to your first visit:

Depending on your agreement with your insurance provider, you may be required to pay a copayment, coinsurance, and/or deductible at the time of your visit. We accept cash, check, bank draft, or credit card
(MasterCard, Visa, and Discover) as form of payment. For more information, please review our Patient Financial Policy.
You can expect your first visit to last approximately one to two hours, depending on the nature of your visit. Please be aware that in order to fully and properly examine you, we are required to dilate your pupils. Dilation can cause your eyes to be sensitive to light, and you may experience slightly distorted vision. These side effects typically do not last more than three to four hours. For this reason, you may wish to have someone drive you to and from your appointment. You may also wish to bring a pair of sunglasses, or you may request a pair of disposable sunglasses upon check-out.
